Methodology & limitations

Every number below is computed live from AccessiSight's own scans table, via a database function scoped to return only a violation's rule id, its severity, and two counts. It never returns a URL, a scan id, or anything that identifies who ran the scan or which site it was.

This is not a random sample of the web. It reflects sites people have chosen to scan with AccessiSight, which likely skews toward site owners who already care about accessibility. A random crawl (like WebAIM's annual Million report , which audits the top 1,000,000 homepages regardless of consent) would likely find a higher failure rate than what's shown here, not a lower one.

Figures refresh automatically as our scan volume grows. There is no manual publishing step, and no editorial control over which numbers appear.

Most common violations

Based on 48 completed scans. Ranked by how often each rule fired across every scan.

  1. 1

    Target Size (Enhanced)

    moderateWCAG 2.5.5 (AAA)

A touch/click target measures under the 44×44 CSS pixel floor at a SPECIFIC responsive breakpoint (375/768/1024/1440px), even if it meets the floor at other screen widths - a single-viewport check cannot see a regression that only exists at one breakpoint.

Found in 46 scans (57 total occurrences)

  2. 2

    Bypass Blocks

    moderateWCAG 2.4.1 (A)

Page must provide a mechanism to skip repeated navigation blocks and jump directly to the main content.

Found in 39 scans (40 total occurrences)

  3. 3

    Target Size (Minimum)

    moderateWCAG 2.5.8 (AA)

An interactive element measures smaller than 24×24 CSS pixels, below the minimum operable target size for users with motor disabilities.

Found in 31 scans (31 total occurrences)

  4. 4

    Page has no main landmark

    moderate

The page has no <main> (or role="main") landmark. Landmarks let screen reader users jump straight to the main content; they are recommended, not required by WCAG.

Found in 29 scans (29 total occurrences)

  5. 5

    No Keyboard Trap

    criticalWCAG 2.1.2 (A)

Keyboard focus became trapped during active tab traversal simulation - a keyboard user cannot navigate away.

Found in 28 scans (28 total occurrences)

  6. 6

    Focus Visible

    criticalWCAG 2.4.7 (AA)

An interactive element's focus style changes in CSS, but the before/after screenshots show no visually perceptible difference, so keyboard users can't see where focus is.

Found in 18 scans (25 total occurrences)

  7. 7

    Reading Level

    moderateWCAG 3.1.5 (AAA)

Content should be understandable by users with cognitive disabilities. This flags pages whose Flesch-Kincaid grade level exceeds a 12th-grade reading level - WCAG 3.0 (draft) COGA guidance builds on this same 3.1.5 requirement.

Found in 24 scans (24 total occurrences)

  8. 8

    Focus Visible

    criticalWCAG 2.4.7 (AA)

An interactive element shows no visible change (outline, box-shadow, border, background, color, or text-decoration) when it actually receives real keyboard (Tab) focus.

Found in 20 scans (20 total occurrences)

  9. 9

    Link Purpose (In Context)

    seriousWCAG 2.4.4 (A)

Link text must clearly communicate where the link leads without relying on surrounding context.

Found in 19 scans (19 total occurrences)

  10. 10

    Contrast (Enhanced)

    minorWCAG 1.4.6 (AAA)

Text falls short of the AAA-level 7:1 contrast ratio (4.5:1 for large text) - a stricter threshold than the AA 4.5:1/3:1 minimum, intended for content targeting users with low vision who need enhanced contrast.

Found in 17 scans (17 total occurrences)

  11. 11

    Abbreviations

    minorWCAG 3.1.4 (AAA)

Abbreviations and acronyms must have their expanded form available (on first use or via <abbr> element).

Found in 15 scans (15 total occurrences)

  12. 12

    Bypass Blocks

    moderateWCAG 2.4.1 (A)

No skip navigation link found on this page. Keyboard users must tab through all navigation links on every page.

Found in 14 scans (14 total occurrences)

  13. 13

    Language of Page

    seriousWCAG 3.1.1 (A)

The default human language of each page must be programmatically determined.

Found in 12 scans (12 total occurrences)

  14. 14

    Page has no h1

    moderate

The page has no visible <h1> element at all, leaving screen reader users with no single, unambiguous entry point when scanning the page's heading list.

Found in 11 scans (11 total occurrences)

  15. 15

    Page Titled

    seriousWCAG 2.4.2 (A)

Web pages must have a descriptive title that identifies the topic or purpose.

Found in 11 scans (11 total occurrences)
